Online Application Instructions for an Initial Physician Assistant License

The Physician Assistant Board will expedite the licensing process with the required documentation for:

  • Active Duty Military
  • Military Veteran – Honorably Discharged
  • Active Duty Military – Spouses or Partner

PLEASE REVIEW THE APPLICATION INSTRUCTIONS AND GENERAL INFORMATION BEFORE COMPLETING THE ONLINE APPLICATION.

ONLINE APPLICATIONS SUBMITTED WITHOUT PAYMENT ARE NOT CONSIDERED RECEIVED AND WILL NOT BE PROCESSED.

PLEASE ALLOW THE BOARD 30 DAYS TO PERFORM AN INITIAL REVIEW BEFORE CONTACTING THE BOARD FOR AN UPDATE ON AN APPLICTATION STATUS.

In addition to submitting the online application, the following items MUST be submitted to the Board by mail:

  1. Form PA5 – License Application Attestation.Original signature only; electronic or printed signatures are not acceptable.
  2. Form PA6 – Physician Assistant Training Program Certification.
  3. Form PA7 – Verification of Licensure as a Physician Assistant or Other Healthcare Professional, if applicable. This is a courtesy verification form to initiate the request to verify a license; the Board will accept other states’ verification letters.
  4. Form PA8 – Birth Month Licensing Request, if applicable.
  5. PANCE Score
    Please visit www.nccpa.net to authorize the release of your score electronically to the Board. California does not require Physician Assistants to be NCCPA certified in order to practice in California therefore licensing is based on the PANCE exam score. Certification letters and PANRE score letters will not be accepted.
  6. National Practitioner Data Bank Report
    Please visit www.npdb.hrsa.gov to submit a self-query report request. Please mail the original sealed report to the Board; PDF versions of the report will not be accepted.
  7. A completed Live Scan Service form or two completed fingerprint cards. Additional information regarding fingerprint cards processing is available at https://www.dca.ca.gov/webapps/pac/fp_requests.php.

As an applicant, you are personally responsible for all information disclosed on your application, including any responses that may have been completed on your behalf by others. An application may be denied based upon falsification or misrepresentation of any item or response on the application or any attachment.
